Documents relating to the medical, educational, personal, and legal background of each youth admitted to the various schools of the Department of Youth Services (DYS). Types of records contained in the series include legal documents, such as court orders, court social history forms, petitions and affidavits; personal data listed on face sheets; and medical reports. In addition, these files contain social, psychological, educational, and vocational evaluations and recommendations. Previous evaluations prior to DYS custody are included in the file, along with copies of school records, such as transcripts. Lastly, there are individual educational plans (IEPs), progress reports, behavior summaries, disiplinary reports, admission of rights forms, and related correspondence.
